Request for Proposals

The Southwest Indiana Area Health Education (SWI-AHEC) is pleased to announce a request for proposals for projects addressing the specific objectives of the Southwest Indiana AHEC Program. SWI-AHEC goals are to improve the number and distribution of the health professions workforce, to improve the diversity of the health professions workforce, and to improve the quality of health professions education and healthcare services in Indiana. 

Objective A: (KIDS INTO HEALTH CAREERS) Improve knowledge and awareness among precollege and college students, school counselors, and parents in medically underserved communities (MUC) of health professions career opportunities, with special emphasis on underrepresented minority and disadvantaged (URMD) pre-college and college students.

  1. K-16 Kids into Health Careers awareness activities less than 20 contact hours
  2. K-16 Kids into Health Careers awareness activities more than 20 contact hours

 

Objective B: (CLINICAL EXPOSURE) Increase exposure of health professions students
to medically underserved communities and other health professions disciplines.

1. Clinical rotations
2. Interdisciplinary training of health professions students
3. Service learning

Objective C: (QUALITY IMPROVEMENT) Enhance access to continuing education in the health professions and to clinical information resources that are responsive to the needs of clinicians caring for medically underserved communities and the health care needs of those communities.

Submitting an application: Applications must be submitted by October 30, 2009 at 5pm. Applicants must submit a cover page, program narrative, and budget with budget narrative. Applications may be submitted in hard copy (4 copies) or in electronic copy to Jennifer L Craig, Director, SWI-AHEC/University of Southern Indiana, 8600 University Blvd, Evansville, IN 47712, telephone (812) 461-5446 or email jlcraig3@usi.edu.

Geographic area: Services under this RFP are limited to one or more of the following SWI-AHEC service area counties: Daviess, Dubois, Gibson, Knox, Perry, Pike, Posey, Spencer, Vanderburgh, and Warrick.

Funding: Proposals are not to exceed $10,000. At least 20% of the financial support must come from source other than grant, in-kind contributions will be accepted. A letter of certification of auditable cost-share expenses will be required at the end of the program period. AHEC staff may request copies of supporting documentation from grantees for all grant and cost shared expenses. Partnerships are encouraged by SWI-AHEC.

Grant Announcement & Publicity: Applicants must acknowledge SWI-AHEC as providing funding for programs on all publications and media coverage related to the funded program. Where appropriate, SWI-AHEC’s official logo should be included (this logo is available in electronic format upon request). A copy of all promotional, media, and educational materials will be requested with the Final Report.

Evaluation: Grantees must participate and assist with AHEC evaluation activities. SWI-AHEC will provide all forms, upon proposal acceptance, necessary to collect required data on programs and participants.

Final Report: A Final Report is due by August 31, 2010. A Final Grant Report Form will be provided from SWI-AHEC upon acceptance of proposal.

Program period: All programs must be completed and invoiced to SWI-AHEC by July 31, 2010. All funds will be awarded as a one-time cost reimbursement payment upon receiving the invoice.

Review process: A review sub-committee of members from the Southwest Indiana regions will review applications and make recommendation to SWI-AHEC. Proposals will be evaluated based on six categories: Service Area, Program Participants, Evaluation, Proposed Budget and Justification, Proposed Local Contributions. Each category will be weighted equally. These criteria are explained in detail in Appendix.
