Advancing Desired Case Management Outcomes

OVERVIEW

To identify tools that will foster a more effective delivery of case management services and promote a renewal of the passion that is an essential element of each case management practice.

OBJECTIVES

Case management is no longer just a cost containment tool; solely relegated to the Work Comp or Managed Care Environments. Today?s case manager functions in every healthcare setting through the entire healthcare continuum. And, although practice settings may differ, each case manager is responsible for advancing the delivery of quality healthcare services for their patients.

As case managers work to facilitate the delivery of that care, they are also challenged to evaluate the cost effectiveness of healthcare services and promote patient advocacy. So, the case manager endeavors to foster the careful shepherding of healthcare dollars while maintaining a primary and consistent focus on patient advocacy.

The achievement of these case management goals can be difficult in today's healthcare environment since that environment is strained by costs that are increasing by over 10% per year; challenged by demands for advances in technology that require an even greater utilization of healthcare dollars; restricted by availability of healthcare professionals to deliver that care; and, choked by an aging population that requires more and more healthcare services.

Even with all these challenges, case management has emerged as the one strategy that balances fiscal responsibility and patient advocacy. But, to be successful, case managers must have the tools that foster achievement of desired case management outcomes.

This program will seek to identify and advance "tools" that will foster a more effective delivery of case management services and promote a renewal of the passion that is an essential element of each case management practice.


Program Objectives:

1. Identify appropriate definitions of case management and detail the common roles and functions of case management.

2. Understand the current state of case management in today's healthcare environment.

3. Develop strategies for advancing the achievement of timely and appropriate advancement of patients through the healthcare continuum.

4. Identify "tools" that advance desired discharge outcomes.

5. Detail specific benchmarks that confirm the efficacy of case management interventions.
