AND INFANT OUTCOMES



Problem:Vanderburgh County FIMR review Board
found referral for genetic counseling was
rarely documented on the cases where a
fetal or infant death was caused by a
genetic defect. Board members
themselves were not up-to-date on
genetic resources in Evansville.

Problem:Half of the fetal and infant deaths
occurring in Vanderburgh County
hospitals are from families who are
nonresidents of the county. FIMR Board
review of the cases found that
documentation of the care and/or
services delivered by the home provider
and facility were missing from the local
medical records. This issue has an
impact on documentation as well as
possible follow-up of health care needs.

Practice Tip:When a pregnant mother or infant are
transferred to another facility, the
mother's prenatal record, history and
physical, labor and delivery record, and
any other pertinent medical records
should be copied and either FAXED or
transported with the patient.
