In order to make sure that you are on track for graduation and to complete
the required forms, the semester prior to your anticipated graduation
date, you need to do the following:
- Make an appointment and meet with Rohn Butterfield, HS Faculty Member,
to complete the Formal Application for Graduation Form.
(Appointments available on Wednesdays from 11 am - 4 pm).

want USI to mail your diploma to you or if you prefer to pick it up
