A student wishing to apply to the Bachelors of Science in Health Services (BS HS) Program must apply to both the University of Southern Indiana and to the Health Services Program.

2. Three forms are required to be submitted in order to apply to the BS HS program at USI. The forms below may be filled out electronically, printed out, and then signed and submitted together to the address below:
