Application Process Checklist

• Complete all required prerequisite courses.

• Earn a cumulative GPA of 2.8 or higher and a grade of "C" or better in each prerequisite class.

• Submit an application & be accepted to the University of Southern Indiana.
  • You may apply online at http://www.usi.edu/admissn/apply.asp. Your dental hygiene application will not be considered for admission without applying to USI also.

• Submit an application to the USI Dental Hygiene Program by February 1^st of the calendar year you desire admission to the program.
  • Download an application: dentalhygieneapplication.pdf.

• Submit official university transcripts from any university other than USI to the dental hygiene program.
  • (USI transcripts need not be submitted).

• Submit official university transcripts from any university other than USI to the admissions office for evaluation.
  • (USI transcripts need not be submitted).

• For transfer students:
  • As you complete each semester, make sure the dental hygiene program and the University receive updated transcripts.
