Application Process Checklist
- Complete all required prerequisite courses.
- Earn a cumulative GPA of 2.8 or higher
and a grade of "C" or better in each prerequisite class.
- Submit an application & be accepted
to the University of Southern Indiana.
- Submit an application to the USI Dental
Hygiene Program by February 1st of the calendar year you
desire admission to the program.
- Submit official university transcripts
from any university other than USI to the dental
hygiene program.
- (USI transcripts need not be submitted).
- Submit official university transcripts
from any university other than USI to the admissions
office for evaluation.
- (USI transcripts need not be submitted).
- For transfer students:
- As you complete each semester, make
sure the dental hygiene program and the University receive updated
transcripts.
