Application Process Checklist
- Complete all required prerequisite courses.
- Earn a cumulative GPA of 2.5 or higher
and a grade of "C" or better in each prerequisite class.
- Submit an application and be accepted
to the University of Southern Indiana.
- Submit an application to the USI Dental
Assisting Program by March 1st of the calendar year you desire
admission to the program.
- Submit SAT or ACT scores, official transcripts from high school, and any university other than USI to the Dental Assisting Program.
- (USI transcripts need not be submitted).
- Submit official transcripts from any
university other than USI to the admissions office
for evaluation.
- (USI transcripts need not be submitted).
- Submit the Dental Office Observation Form to the Dental Assisting Program.
- A minimum of 8 hours of observation is required.
